GENERAL INFORMATION
**IMPORTANT NOTE: If you have any questions regarding player waivers, LOR release forms or player selection, have questions in regard to why your player was not selected for a specific team, please contact the Head Coach. The APMHA will not meet regarding these requests. Forms are only granted once the coach has released the player from the tryout process.

TRYOUT FEES
Tryout Passport - allows players to attend tryouts until they are selected or released at a particular level.
(ex. If a player skates at a AAA tryout and is unsuccessful, they are able to skate at the AA tryout without having to register again or pay additional costs for those tryouts). Once released by the coach at a particular level, players may not return to that level for additional tryouts. Tryout passports must be purchased online during the registration process (payment can be made using Visa/Visa Debit/Mastercard).
*Early Bird Pricing* - Tryout Passport Fee Up to & Including April 5th - $70.00
Tryout Passport Fee April 6th & onward - $90.00
Pay at the Door - $35 per each individual skate/tryout.

**IMPORTANT** - The players name must appear on the tryout registration list.
All players must register for tryouts prior to stepping on the ice. Tryout Passports are non-refundable.
A completed tryout registration form will create a PTN (Personal Tryout Number). This PTN will follow you/your player throughout the tryouts and be used to identify players moving on to additional tryouts. You only need to register for tryouts and complete this form ONCE (ex. If you are unsuccessful at AAA and move to AA tryouts, or AA to A, the players PTN will be passed on to the coach of the next level, no need to fill in another form).

***Upon signing the Offer of Commitment, the APMHA association registration and registration fee is due immediately. Registration must be completed using the APMHA online registration store and payment can be made via cheque, Mastercard/Visa or e-transfer.
