If it is determined that you require a 'AAA' Passthrough Waiver, please complete this form. AAA Waiver Requests will only be accepted via this form. Please allow 48 hours for this form to be processed. Requests received with incomplete information will take longer. Once approved, you will receive the AAA Waiver electronically from the OMHA.
